  • For students – provide death certificate, place of residence and where student grew up, student number, year and area of study, photograph, cause of death, memorial/funeral service details (if available) 
  • For staff – department to provide two or three sentences (approx 250 words) on staff member’s role, achievements etc, photograph, cause of death, memorial /funeral service (details if available)
